Conference Registration Form

The Conference Registration Form is available in Postscript (18k) and RTF (63k) formats. You cannot register directly via the Web or email. However, you can download the form, print it out, complete it, and fax or mail it to the CHI 96 Registration office. The form includes information on where to send your completed form.

Those who wish to pay in Canadian currency may do so by sending a personal check, company check, or money order, drawn on a Canadian bank, to the CHI 96 Registration address listed on the registration form. Registration forms in Canadian currency are available in Postscript (18k) and RTF (63k) formats as well. Credit card transactions should use the "US Dollar" forms above--the credit card service will be used to convert to Canadian currency.

If you have any problems printing any form, please contact chi96-webmaster@acm.org. You may have to set your left margin to under 0.5 inch (1.25 cm) to get the RTF format to print on just 1 page. Also, the RTF format may print a blank second page.
